Where to find a PAD/DD form on your online banking

BMO Bank of Montreal (001)

Click on My Accounts.  

Select your bank account.

Click on Void Cheque. For Small Business customers, click on Re-Order Cheques.

(The void cheque link is near the top, in the blue summary of account details area).

Sign into your online banking

Click on your Account

Click on the "Show account number and details" link

A sample cheque will be shown


ROYAL BANK (003)

To print your customized form you will need to:

Sign In to Online Banking

Click on any of your accounts.

Select “View and Print Payroll Direct Deposit” from the right navigation.

Select the account you want to deposit your payroll to from the dropdown, then click on “View and Print” and your customized form will be presented to you.

Simply print, sign and submit a copy to your employer’s payroll department and your part is complete!


TD Canada Trust (004)

The Direct Deposit form is only available via EasyWeb. After you log in to EasyWeb.   

Click on the account for which you require Direct Deposit information.

Once on the Account Activity page, look to the far right of the Current Balance header.

Click on the "Direct deposit form (PDF)" link.

You will then see a pop-up page (PDF) with your Transit (branch) number, Institution Number (for TD Canada Trust) and your Account Number pre-filled for the direct deposit.

Use the account details provided to set up Direct Deposits or Pre-Authorized Debit Payments.


CIBC (005)

Sign into CIBC Online Banking and select “My Accounts” from the menu.

Select the account you want to link.

Select the “Void Cheque / Direct Deposit Info” link in the account details.

Review the Account Information form with your account information including transit number, institution number, and account number.

Print the form, add your signature and date, and submit to the company who requested the information.
